Dr. Adam Bonnington, MD is a Gender Surgeon in San Francisco who has focused his career on working with underrepresented patient populations. He joined Dr. Heidi Wittenberg at MoZaic Care as a Surgical Associate in 2020 where he works exclusively with transgender and non-binary individuals. Dr. Bonnington performs Orchiectomy and Vaginoplasty.

Dr. Bonnington performs a few different types of Vaginoplasty procedures:

Penile Inversion Vaginoplasty: Referred to as the "gold standard" in gender-affirmingVaginoplasty techniques, Penile Inversion is often performed as a one-stage procedure, incorporating Orchiectomy, partial Penectomy, penile dissection and creation of the vaginal cavity, labia and clitoris. For patients who have a limited amount of tissue left to construct the inner labia and clitoral hood, Labiaplasty and Clitoroplasty are completed in a subsequent surgery.

Peritoneal Vaginoplasty: This procedure uses abdominal (peritoneal) lining for the creation of the neo-vagina. Related procedures--labiaplasty, clitoroplasty, penectomy, orchiectomy and partial urethrectomy--are similar to that of the Penile Inversion technique. As a primary technique, Peritoneal Vaginoplasty is best-suited to patients who have atrophic anatomy (such as the result of having been on puberty blockers) or otherwise need more tissue than what would be available with Penile Inversion Vaginoplasty, and those who are seeking a non-binary procedure, such as Penile Preservation Vaginoplasty. Peritoneal Vaginoplasty is also done as a revision or repair surgery in cases where the vagina has closed or requires more vaginal depth.

Penile Preservation Vaginoplasty: With this procedure, the vagina can be created using a scrotal or other skin graft, or peritoneal tissue, while the penis is retained. There are some trade-offs: Vaginal depth with only a scrotal graft may be insufficient, and with a peritoneal graft, the testicles and scrotum could be kept, but there would be less room and less skin available for the vulva. The urethra can be re-routed or left in the penis. Labiaplasty is optional and often done 6-12 months after the initial surgery.

Dr. Bonnington also performs Vaginoplasty repair surgeries. Complications after Vaginoplasty can include urethral strictures, fistulas and abscesses. Dr. Bonnington can provide surgical solutions for these conditions and provide long term follow-up care for patients who have had Vaginoplasty.

Dr. Bonnington follows WPATH guidelines for surgery. You will need to provide two referrals from qualified mental health professionals, as well as a letter from your hormone specialist stating you have been on 12 months of continuous hormones (unless clinically contraindicated.) Patients must be 18 or older to undergo irreversible genital surgery procedures.

Dr. Bonnington accepts insurance from a wide number of insurance companies. A cash pay rate can also be requested.
